diagnosis or for intermittent driveability conditions. This form should also be attached to the repair order.
If properly completed, these forms can help to quickly and accurately diagnose and repair vehicles with driveability conditions. These forms can also
provide CAD-STAR with valuable vehicle data and contain all of the necessary information to be sent via a facsimile (FAX) machine. When detailed
symptom, code set, or snapshot data is requested during an ongoing CAD-STAR case, you will be instructed on how to send the completed forms. These
forms do not eliminate the need to fill out the CAD-STAR Pre-Call Worksheet, available from Helm, Inc., form number H-8645R.
