When contacting Exchange Center for SDS, dealer must provide the correct model number and description of radio.

Advise the customer that a replacement radio has been ordered and will be installed the same day.

Remove suspect radio from vehicle and prepare an Equipment Manufacturer's Claim Request (form #84-581-7816), ensuring all pertinent
information (as shown on pages 9 and 10), including WRO number, is filled out and radio and EMCR are ready for SDS delivery person upon
arrival.
6.
Signify that the SDS transaction occurred within the specified time frame by writing "SDS" and signing and dating page 2 of the EMCR in the box
marked "For Chrysler Use" as shown on page 9.
