NOTE:
Under Program 06N05 each vehicle is limited to one replacement headlamp switch knob.
Submission of a claim under 06N05 will remove the vehicle from OASIS; the affected vehicles are not eligible for multiple repairs once the headlamp
switch knob has been replaced.

STOCK VEHICLES
Do not perform this program unless the affected vehicle exhibits the covered condition (fix only if broken).
SOLD VEHICLES
Only owners with affected vehicles that exhibit the covered condition will be directed to dealers for repairs.
TITLE BRANDED / SALVAGED VEHICLES
Affected title branded and salvaged vehicles are eligible for this Field Service Action.
RELATED DAMAGE
Related damage claims are not approved for this program.
ADDITIONAL LABOR TIME
This repair will require no additional labor time to complete. Claims submitted with "MT" labor will not be accepted for reimbursement.
OWNER REFUNDS

Ford Motor Company will only refund owner-paid headlamp knob replacement if the repair was performed prior to the date of the Customer
Notification Letter (or after the date of the letter if an emergency repair was made away from the servicing dealer.)

Ford Motor Company will also refund owner-paid headlamp switch replacement if caused by headlamp knob failure during the time period when
headlamp knobs were not serviced separately.

Refund claims that include other non-covered repairs or those judged by Ford to be excessive will not be accepted for reimbursement.
