Order new seat(s) directly from Johnson Controls using the fax form shown.

2.

Upon receipt of new seat(s): Replace seat(s), referring to WIS document AR91.12-P-1000RT.

Return seat(s) removed from vehicle to Johnson Controls (not the QEC) using packaging from replacement seat(s).
