All parts must be returned with a Form FPS-340, "Customer Request for Credit on Return Parts and Accessories" coded L "Campaign Parts". To receive,
full credit to your parts account, the Form FPS-340 must be annotated "RETURNED PER INSTRUCTIONS IN OWNER NOTIFICATION
PROGRAMS M24/M25, and must have a copy of the packing slip attached. Credit to your parts account will cover normal freight and parts costs.

CLAIM PREPARATION INSTRUCTIONS (A copy of these instructions should be filed in Subject 13.0 of your Warranty and Policy Manual).
Repairs performed under Owner Notification Program M24 must be authorized by your Ford Parts and Service Division District Office. They may be
submitted on warranty claim Form 1863, which is to include the information shown below. Enter Program Number M24 in the Program Code box in the
upper left of the form. Repair claims should be submitted through the Direct Warranty Entry (DWE) System.

Owner reimbursement requests submitted under Owner Notification Program M25 are governed by the following guidelines:

Owners seeking reimbursement will be required to present their paid repair order or over-the-counter parts purchase receipt to any Ford or
LincolnMercurv dealership by May 31, 1984.

A copy of the paid repair order showing the date and vehicle mileage at the time of the repair or an over-the-counter parts purchase receipt must be
retained in your service files for one year from the date of claim payment.
^
To obtain reimbursement, you may submit individual owner reimbursement requests by mail. You must submit such request on a separate claim
Forms 1863 by July 31, 1984. Claims submitted after July 31, 1984, will not be paid.
