Dealers must request 3rd party QC inspection immediately following completion of campaign repairs (P5137). These inspections will be conducted by
Integrated Warranty Services, Inc. (IWS). This requirement will stay in effect until further notice.

Requests for QC inspections may be made by completing the IWS Inspection Request Form included in this bulletin and faxing it to (310) 323-3636.
Please photocopy the IWS Inspection Request Form as needed to ensure an adequate supply of forms for future QC Inspections. Inspectors will be at the
dealership within 36 hours.
