The dealer should complete the attached order form and secure the customer's signature. Make two photocopies of the form; keep one for your
dealer's records and give one to the customer. Mail the original form to the Consumer Affairs Department.

VCNA/PD will ship the part to the dealer on a "drop ship" basis.
