The attached page contains a new vehicle glass, stress crack/distortion survey which must be completely filled out and Faxed to the number listed on the
survey any time a windshield is replaced on a vehicle within the applicable new vehicle warranty.
Prior to replacing a windshield for stress cracks or distortion:

Verify stress crack conditions using a magnifying glass and carefully inspect windshield along the crack to ensure the cause is not from outside
influence such as stone chip or road hazard.
Note:
Windshields with damage from outside influence are not a warrantable item.

Make copies of the attached survey form, fill it out completely and Fax it to the number listed, any time a windshield is replaced within the applicable
new vehicle warranty.

OTS approval is required before replacing a windshield.

A copy of the completed survey must be attached to the Repair Order.

Parts requested must include a copy of the completed windshield survey.
