REIMBURSEMENT

Only the Form 1864 will be accepted for reimbursement
under this program. Upon completion of the required service, the Form 1864 must be completely filled out and signed by
authorized dealer personnel.  Mail the second copy of the Form 1864 for reimbursement.

It is important that all claims be filed for all completed units so that you will be reimbursed and our records will show
completion.

Inspect seat belt webbing and install cover.

0.2 Hrs. - Insert in Box "A" on Form 1864

Install New Seat Belt Assembly (includes inspection of the seat belt webbing and installation of cover)

0.7 Hrs. - Insert in Box "B" on Form 1864

Administrative Allowance

0.1 Hrs.

NOTE:

Add 0.1 hour Administrative Allowance to the total repair time.

IF THE SEAT BELT IS TO BE REPLACED, DO NOT SUBMIT FORM 1864 UNTIL THE BELT HAS BEEN INSTALLED.

PARTS REQUIREMENTS

IN FIES DIRECT MARKETS, WHERE PERMISSIBLE, PIVOT COVER AND PUSH PIN (E7F2-5861693-B) WILL BE DIRECT SHIPPED TO
OWNERS WITH THE OPTION THAT EITHER THE OWNER OR DEALER MAY INSTALL IT.  (PUSH PINS ARE INCLUDED WITH PIVOT
COVER).  REMAINING PIVOT COVERS WILL BE DIRECT SHIPPED TO FACING DEALERS AND AFFILIATE FORD COMPANIES
ACCORDING TO VEHICLE QUANTITIES SHOWN ON VIN LIST.  NOTE:  ALL PIVOT COVERS AND PINS ARE BLACK IN COLOR

Inspect seat belt assembly for any visible sign of damage before ordering new belt assembly.  Seat belt kits will not be direct shipped for this recall.
