SERVICE
If the owner of an eligible vehicle does not have a pre-printed
REIMBURSEMENT
Form 1864, you must use a blank Form 1864. Only the Form 1864
FORM 1864
is acceptable for reimbursement on this program. The Form 1864
must be completely filled out and signed by authorized dealer personnel. Submit the second copy of the Form 1864 for
reimbursement. Claims must be submitted for all completed units.
NOTICE
Owners may inform the National Highway Traffic Safety
Administration if the recall is not performed without charge or in a reasonable time after the vehicle is brought to your
dealership for repair.
PARTS
An initial supply of retainer clips are being direct shipped to
dealers. FIES will direct ship to dealers quantities of retainer clips based on individual dealer sales volumes. These direct
shipments will continue until further notice. Fuel line retainer clips will be provided at no charge.
