REPORTING COMPLETIONS
Only the Form 1864 will be accepted for reimbursement under this program. Upon completion of the required service, the Form 1864 must be
completely filled out and signed by authorized dealer personnel.
CLAIM SUBMISSION
The claims must be entered through the Direct Warranty Entry (DWE) system. Dealers without DWE capability must mail the second copy of the Form
1864 for reimbursement. Should your dealership receive pre-printed Forms 1864 with a seven digit claim number, you must drop the first digit and enter
only the last six digits. You must sight verify the claim number for accuracy. It is important that all claims be filed for all completed units so that you will
be reimbursed and our records will show completion.

For 1985 and 1986-Model Manual Transmission Equipped Vehicles With Calibration 6-07S-R12 (Kit Codes A and B)
1.
Remove the existing vacuum delay valve (# 2, Brown/Black) in the choke pulldown vacuum line and replace with a new stepped straight vacuum
line connector (Part Number 383004-S).
2.
Install a vacuum delay valve (# 10, Yellow/Black, Part Number D3AZ-12A091-A) in the vacuum line connected to the distributor vacuum
diaphragm using a 3-inch length of vacuum hose (5/32" ID, Part Number D0PZ-12226-B). Assure that the black side of the delay valve is
positioned toward the vacuum source.
3.
Replace the existing EGR orifice connector (6.5 mm, Brown) with a new EGR orifice connector (7.0 mm, Red, Part Number E4FZ-9F485-D). To
make the replacement easier and prevent damage, the EGR tube should be loosened at the EGR valve.
4.
VEHICLE EMISSIONS CONTROL INFORMATION (VECI) DECAL
Obtain the decal from the owner and verify that the vehicle identification number (VIN) on the decal matches the VIN of the vehicle. Attach the
