CLAIMS INFORMATION:

Service Procedure

Confirm the vehicle front tires are OEM Bridgestone 225/50R17 Potenza RE040, or Bridgestone 225/45R18 Potenza RE040 originals or
replacements.

If the vehicle has any tires other than above it is not eligible for this offer. No further inspection is needed.

2.

Check the vehicle mileage:

Confirm the mileage on the vehicle's current set of OEM size Bridgestone front tires is fewer than 14,000 miles.

The total mileage on the current set of front tires can exceed the 14,000 limit up to a permitted grace period of 1,000 miles per 30 days of use
after the issue date on the Service Visit/Tire Discount Coupon, up to a maximum of six months (6,000 miles) when the coupon expires.

If the mileage on the tires is over the limit (with applicable grace period), the vehicle is not eligible for this offer. No further inspection is
