Use the form and complete with your Dealer Code, OLD Timing Belt parts quantity, total dealer cost for all OLD Timing Belts that were removed from
inventory and destroyed/scrapped. Your ZSPM/DSM/PC must approve the reimbursement form and a credit will be processed under a separate invoice
identified in the Isuzu Statement of Account.
